Do I need a single-groove or multi-groove pulley?

Match the pulley to the belt your engine already runs. A single V-belt needs a single-groove pulley, and a serpentine or multi-groove belt needs the matching multi-groove pulley. Count the grooves on your old alternator before you order.

Is my alternator actually bad, or is it something else?

Low charging is not always the alternator. A slipping belt, loose or corroded output and ground connections, or a tired battery can look the same. Check those first, and replace any corroded cable or lugs with tinned battery cable while you're in there.

Should I replace the belt when I replace the alternator?

If the belt is glazed, cracked, or stretched, yes. A new alternator on an old belt can still slip and undercharge. Set belt tension to the engine maker's spec after the install.
